Accessories Tempered Glass Screen Protector Suggestions?

I have a tempered Glass protector on my phone, but it came from Verizon and I can't even remember the brand.

Looking at Amazon I see a ton of protectors with mixed reviews.

I'm looking for one that is less than $20 USD, easy to apply, durable, and had a lifetime unlimited warranty (replacement for cost of shipping, no questions asked).

Anyone have any suggestions?
