The "UNKNOWN" when someone calls - how to disable?

This has been bugging me for many months.
What is the black bar that says UNKNOWN when someone calls and how do I get rid of it?
Their phone number (and name, if in Contacts) is displayed but this black bar is also displayed.
It is so obnoxious and pointless.
How do I disable it?

There should be a settings in your phone app that you can turn on to show contact info.
