To check if it's rooted you could install root checker. It will verify if the device is rooted.
Unless it's a free low end monitoring app it will require root access.
Free apps will show under security then admin. Paid apps will not show there. They will install named X app and once installed and granted root, they will hide themselves as usually a setting app.

Do you suspect this might have happened with it. Did you buy it second-hand, has it ever been out of your possession, is it a company phone, etc?
