Help Translucent phone icon on screen

[Android Question]

There is a translucent image of a phone appearing on the screen of my husband's Galaxy S5. This just appeared yesterday and when he touched it, his phone locked up. He had to remove the battery to reboot. Any idea what this is and how it got on his phone?
