I searched the threads and the forums and the only fix people have is hard rebooting and sound manager. Neither of which has worked for me. I also did a nandroid restore back to gingerbread and still no vibrations on anything... all my settings are turned on. i hold the volume button down to were it would go to vibrate, but it doesnt vibrate. It only shows vibrate mode.

Also it no longer vibrates when I start the phone.

    Things i have tried...

    Flashed different roms
    wipe date/dalvic/cache
    sound manager
    adjust all settings
    hard resets

    is there anything missing?

or do you guys think the hardware is the issue?

sounds like it may be a hardware issue but lets try this

go back to settings and uncheck everything you have selected with vibrate. then recheck them reboot wipe cache/dalvic and reboot and see what happens you should be able to press the volume down key and see if the phone vibrates when you push the key down to vibrate for Ringer Volume.
