Help Issues with Micro SD card

I have looked everywhere and there doesn't seem to be a solution or I'm just not looking in the right places. Ive been on lollipop since it was released and up until a couple of weeks ago I've had an issue where I'm unable to record or take pictures to to the card and also delete photos (they reappear after I've deleted them). I thought it was a format issue so I backed everything up and tried reformatting but that hasn't worked either. Nothing gets deleted at all. I've noticed on a few occasions when I went to "settings>storage" the card wasn't being read at times. It's only after I either reboot or select reformat that the card becomes readable. Is there a solution to this?

The card shows up on that app. Not sure what else I should look for

I guess it's possible that the card is just getting old.

There's no other way to try to reformat? I'm on a mac so maybe I could do it on there?
