Help Touch inputs producing lines

I have recently purchased a used galaxy note 10.1 and an official S pen separately. For some reason when I am writing with the S pen it will make a line of about 3 mm coming out from the point of contact in a seemingly random direction, I noticed this in multiple apps. From also lightly tapping my screen with a finger it also produces these small lines in a 'random' direction. This does not occur with every touch more around every 5-6 touches.
I was wanting to know if anyone has ever had this problem before. I am not sure if this is due to the limitations of the software or could it be due to the screen.
