it has been so long since my phone looked like that i have forgotten exactly how i did it.
i am sure i was using launcherpro plus as my home launcher. this allows you to add custom icons to the bottom launcher so i made some black text ones to use for messaging, calls, email
the clock i was using is Clockr which you will find on the market, i am not sure what the battery widget is or the signal widget. i also had my notification bar hidden to keep it clean looking.
it was easy to do though and all can be found free on the marketplace except the icons that i made but they are easy to make in photoshop then load them onto your phone and pick them from your gallery.
