Help Touchless Cntrol/ Google Now issue

Just a question, but has anyone had issues with their Maxx doing searches without speaking the launch phrase? During conversations or with the TV on, without uttering "OK Google Now", I'll hear a beep and when I pick up my phone, Google Now is active and wanting me to repeat my command. I believe it started about a week ago, and some comments in the Play Store are saying their are similar issues after the latest update. I've sent an email to Motorola Mobility through the Play Store, but after a week have not received a response. Just curious if others are experiencing this as well, and if they've found a solution. So far all I've been able to do is disable Touchless Controls to resolve it, but it is nice to have that function available.

I have another issue possibly related to the above issue. While driving, and without my phone paired to my car (haven't done the initial pairing yet), during breaks between songs I will hear my phone playing media. In my case it's a movie trailer I have stored on my phone. It happened about 3-4 times while driving around town. It hasn't happened since (the other issue presented itself after the last occurence). Just curious, since in either case I didn't speak the launch phrase.
 
Not sure about the first issue. Try re-training touchless with your phrase. I heard that works for other people that are having this issue. For the second issue, I turned off my "resume music" setting in the Assist app.

I already tried retraining, because if I wanted to use it from further away, my tone changed when I spoke louder, so I spoke regularly and in the same tone as when I raised my voice. But even then it still didn't resolve the issue. Thanks for your help though.
