Samsung GS5 PC connection problems, including no option for USB 3.0

So, went to add some music to my S5 today connect it to my laptop, running windows 8.1 and does the windows ding, shows it is downloading drivers, software whatever, then stops and says that the device was not recognized by my PC. So I check the connection on my phone, and it used to show USB 3.0, USB 2.0 and then the camera option, now it only shows "connected as media device" or the option to switch to camera option? Now when I connect it to the PC, I get nothing, not even the Device not recognized popup. Has something changed in the phone? I am using the OEM cable, and plugged into the USB 3.0 port on my laptop, same one I have always used... having said all of this I have always had issues with my connection to the PC... any help would be greatly accepted.

I have tried the *#0808# option and MTP is selected... I tried MTP + ADP and rebooted, still nothing...
 
I've had no problems on Windows 7 but recently tried on Windows 10 and had the same problem. A web search shows several complaints on Windows 10 and 8.1, so there's something going on with Windows since it doesn't even recognize that the phone has been connected and ding. Installed the latest USB drivers from Samsung which lists Windows 10 32/64-bit as supported but that didn't help.

One poster mentioned that he got it to work with a USB 2.0 cable but not with a 3.0 cable, but haven't tried that yet. I presume that means he used a regular microUSB cable instead of the microUSB 3.0 cable that comes with the phone. The laptop I was trying only has 3.0 ports so another thing I need to try is plugging into a 2.0 port on another Win10 laptop to see if that helps.
