Notification/Button function issues

This morning, I was using my phone, as I normally do. I set it down for a moment and when I picked it up and woke up the screen, the top 1/4 of a screen was black and the bottom 3/4 was the last page I had viewed before my screen went to sleep, only it was pushed down (the top of that screen was under the black part of the screen and the bottom 1/4 was cut off). When this appeared, my device was also frozen. None of the buttons were accomplishing anything. So after waiting several minutes to see if the phone would normalize itself, I finally held down the power button until the phone reset. After restting there have been several issues that I cannot resolve.
1) The home button works only to wake up the screen now, it does not do anything when the screen is actually on. The menu button also doesn't work for anything. Only the back button performs its normal function.
2) My notifications are not showing up either in the status bar or pull down menu. Notifications are enabled and they all make their alert sounds but the icons do not appear. When I pull down the status bar it only shows me a thin bar at the top with date/time, network and battery status. My expand arrow to expose quick settings is gone and no notifications appear here either. I typically have a temperature widget in my status bar, but that no longer appears either. Everything on the right side is there, though (WiFi, Signal, Battery, Time).
3) When hitting the power button to bring up the power options, if the menu is no longer there. Typically it contains options to Power Off, Airplane Mode, Restart Device (and maybe one more? I can't remember). Now it my shows Power Off and Restart Device. The Airplane Mode option (and other?) is no longer available on this menu.

These are just the few issues I have discovered since this happened. There may be more that I'm unaware of at this time. I have restarted the device several times, wiped cache partition, and removed and reinserted battery so far, but none of these have resolved any of the issues. I would like to make sure I exhaust all possible fixes before resorting to factory reset. Any input is welcomed!
 
Actually, once you remove the back cover there are several screws that can be removed to take off another cover to reveal the battery. There is an adhesive film on this secondary cover that states not to remove the film for fear of reducing waterproofing. But I did not remove this. There is a gasket in place under this second cover that I made sure to put back in place properly so as not to compromise any waterproofing. I would advise, though, that the small screws are not of the greatest quality, and I did deform the holes where the screwdriver is inserted. All this being said, I came to 2 conclusions:

1) To be honest, I'm not deep sea diving with my phone, or even taking baths with it, so the waterproofing isn't all that important to me. The phone has since functioned normally when in rain/wet conditions. I guess if the day comes that it does become fully submerged, we will find out then how it performs.

2) I did eventually complete a factory reset of the device and it is back to functioning normally.
