Help HELP PLz random partition=[

hey guys is there a way to unpartition sd card idk how it managed to do after i hung up from call i put my phone in my pocket but a min later i felt a vibrate like it rebooted and when i took out the phone it was on cwm and it had all these errors sd card stuff when i checked my memory stick eveything was gone and now i only can use 11gb of my 16gb memory card=[ is there any way to fix this?

yup formatted sd card by when its in my esteem formatted by usb mode on pc and formatted when i used a secondary sd card carrier nd no luck i only get 11gb=[
