Root Lost FM Tuner when I did Cyanogen Mod

When I did the Cyanogen mod, I lost my FM radio tuner APP.

I know there are all kinds of online radio like iheart, etc. However, I need the FM tuner to access the TV's audio at my gym as well as the play by play at football games where there are 85k people and phone network is slowed to a crawl.

I found an app that claims to be the original FM radio tuner app, but when I try to install, it says I must give it access to make phone calls as well as read my contacts. That scared me a bit, as I can't imagine WHY a radio app would need that.

Anyway, I have two questions:

1. Can anyone tell me if this is a safe app to try:

2. If not, is there a way I can get my FM radio tuner back after I did the Cyanogen mod?
 
When i backed up my phone before i flashed to 4.2 i kept the fm radio. idk if this is the one u need but

Fm Radio

the .rar includes the FMRadio.apk and FMRadio.odex files

The only thing, when I install, it says I have to give the APP access to make phone calls and read my contacts. That concerns me, not sure why a radio app would need to make phone calls. Of course that could just be normal. However, I don't think spirit FM asked for that permission.

I can't even get it to install. Just keeps saying "Application Not Installed".
