sd card missing folders htc evo 4g (SuperSonic)

I formatted my SD card, just got my replacement EVO and thought it would restore folders. How can i get the folders back?

Folders:
Android
Lost.dir
Media
Smwm
Tmp

I remember there being a lot more folders.

Can anyone help me...

EDIT: Also i am new to rooting, but i cannot boot into recovery either on my phone that was just replaced...I dont remember if i was able to boot into recovery before i rooted or not...

Id greatly appreciate any help...
