Notification volume issue

I've just started having an issue with my notification and alarm volume being way too low. Rings seem to be ok, it's just notification and alarm volume. I have it set as loud as it can go, and have the systeme volume as high as it can go and it's much quieter than before and tried different sounds, all with the same result.
I've noticed that that the notification and alarm sound seems to be emanating from the earpiece. I never paid much attention to it before, but should it not be coming from the separate speaker? Obviously, when I put my ear up to the earpiece, the notification sound is very loud.
I have added a couple new apps, most notably Facebook for Android but all was fine after adding it for days. But perhaps the one culprit I suspect is that I updated my Profile on the Hero. Would this have possibly caused an issue like this? Still running 1.5 BTW and did a reboot and pulled the battery, all to no affect.
Also, I'm using Quick Settings and recently updated it, but to be honest, not sure if I noticed the issue after doing so...that was a few days prior to the Profile Update and I think I would have noticed it if QS was the culprit.
 
UPDATE:
I've discovered this is happening to all audio: media, ring tones, alarms, etc. When i play back voicemail and select speaker, nothing happens... I have to put my ear up to the earpiece to hear it.
I remember listening to Scanner Buddy yesterday and when it came on a channel, it was quite loud so I turned it down. Shortly after the phone rebooted itself and from then on is when the problem started. I've tried removing the apps like SB and QS, reboot, pull battery...... all to no avail.
Any thoughts or insights would be greatly appreciated.

Have you tried clearing all cache in manage applications?
