Help Automatically redials while on a call

About half of the time, when I am on a call, the phone will place the current call on hold and redial the same number. I'll be talking and all of a sudden, the person that I am talking to will re-answer their phone (through call waiting or a second line), and my first call to them is on hold.

Has anyone else experienced this? Is it even possible to accidentally cause this behavior, or is this a problem with the phone?
