There are probably some apps that will do it for you, but the easiest way is to root the phone and add the sites you want blocked to the hosts file, pointing them to the phone (127.0.0.1) or to some innocuous site like aol.
To the best of my knowledge there are no longer supposed to be any apps that modify the hosts file allowed on google play. They deemed them to interfere with other apps, and per the the Google Play TOS apps that do so are not permitted.
That said, you could install the app F-droid and get apps like AdAway to import lists into you host file. Or do it manually as the post above me mentioned.
